Functional Nurse Assessments
A Comprehensive Nursing Assessment that is required prior to moving into any Adult Family Home and annually therafter
NWNC Assessments include the state mandated assessment of physical, mental
health and social needs. NWNC Assessments also include:
• Full medication reconciliation
• Review of legal and POLST status
• Review of orders and status
• Narrative Summary, easy to read
• The basis for the care plan to be individualized and pertinent
• Comprehensive Nursing Assessment
• Preliminary Care Plan included with Functional Assessment.
Nurse Delegation And Skin Observation Protocol
Our nursing umbrella provides the initial teaching and ongoing support necessary to provide legal and excellent caregiving in your facility, DDA community setting or private home with caregivers.
Examples of Nurse Delegated Tasks:
• Skin Observation and Pressure Injury
• Insulin Injectables and Blood Glucose Monitoring
• Tube Feeding and Tube Feed Medications
• Oral and Crush Orders
• Drops, ear or eye
• Oxygen
• Seizure Protocols including VNS and emergent management
